Nurse Practitioner

Mobile Medical Group is looking for a Nurse Practitioners to join our team. This is a full- time, salaried with benefits position. Positions available at our Utah, Idaho, Arizona, and Colorado locations.

Responsibilities

• Assists with the provision of palliative care in accordance with facility, state, and federal regulations
• Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs
• Implements interventions to support the patient with chronic conditions to decrease recurring
hospitalizations
• Collaborates with primary care physicians/NP by making appropriate referrals, follow up calls, and coordination of
care
• Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging
patients
• Prescribes appropriate pharmacological and non-pharmacological treatment modalities
• Monitors the effectiveness of interventions
• Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients

Qualifications

  • Will consider less NP experience with RN hospice and/or home care experience

  • Minimum of 5 years OR clinical hospice and palliative care experience. 2 years’ experience in the Nurse Practitioner role preferred

  • Must work well independently and within a team

  • Strong computer skills, including Word, Excel, and PowerPoint

  • At least 2 years NP experience with hospice and/or palliative care experience (either NP or RN)

  • Current Advanced Practice Registered Nurse (APRN) Licensure

  • Awareness of UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful

  • Nurse Practitioner certification in Hospice or Palliative Care within 1 year of starting the position

  • Graduate of an accredited School of Nursing

  • Knowledge of clinical standards of care

  • Education and/or Experience: Completion of Nurse Practitioner accredited program

  • Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care deliver systems

  • Minimum of two (2) years clinical experience, desired

  • Exceptional interpersonal skills & superb communication a must

  • At least one (1) year of experience in Hospice or Palliative Care
